I recently grabbed the Rasterman's rxvt from:
http://www.cse.unsw.edu.au/~s2154962/linux_rxvt.html
This version supports pixmaps in the background. Possibly you could
use the pixmap background as a way of providing the menu-bar you want
to click on. A minor hack to the source code could detect where you
clicked and run the command you want.
