FVWM: FvwmForm

Has anyone had problems getting FvwmForm to run properly? If I try it
from a FvwmTalk window, I get

X Error of failed request: BadValue (integer parameter out of range for
operation)
  Major opcode of failed request: 1 (X_CreateWindow)
  Value in failed request: 0x0
  Serial number of failed request: 26
  Current serial number in output stream: 36

This happens with both fvwm2 (2.0.39) and fvwm2-95 (2.0.41f).

Any ideas?
